Contact Lens Professional Services & Materials Policy

1. The Contact Lens Evaluation & Fitting Process

An annual comprehensive eye examination is required prior to a contact lens fitting.

  • The Fitting Window: The fitting fee includes the initial evaluation and all related follow-up care for up to 60 days beginning from the date of the initial lens application/fitting.
  • Visit Limit: This process covers a maximum of three (3) office visits.
  • Completion: Failure to complete the fitting process within the 60-day window or 3-visit limit may result in additional evaluation fees.
  • Non-Medical Requests: Follow-up visits are intended to ensure fit, vision, and ocular health. Requests for additional visits or brand changes for non-medical reasons (e.g., personal preference comparison) will be billed as a separate office visit.
  • Medical Emergencies: Office visits for red eyes, infections, or other eye emergencies are NOT included in routine contact lens follow-up care and will be billed as medical office visits

2. Supplemental Brand Evaluation Fees (Multiple Brands)

Our tiered fitting fees cover the professional evaluation of one primary contact lens and modality (e.g., one specific daily lens)

  • Multiple Fit Requests: If you request to trial additional brands or modalities for personal preference, price comparison, or to have multiple prescriptions, a Supplemental Brand Evaluation Fee will be charged.
  • Fee Structure: This fee is 50% of the standard tiered fitting fee for each additional brand requested (e.g., if the primary fit is $80, a second brand evaluation is $40).

3. Professional Fees & Materials

  • Professional fees are for the doctor's time, expertise, and data collection. They are non-refundable and do not include the cost of lens materials.
  • Fees for professional services must be paid prior to the ordering of lenses.
  • All lens orders require payment in full at the time of ordering.

4. Specialty Lens (Hard Lenses) & Materials Policy

  • Unclaimed Lenses: Lenses will be held for a maximum of 60 days. It is the patient's responsibility to ensure contact information is current.
  • Specialty Warranties: For specialty lenses, changes in parameters cannot be made beyond the manufacturer’s warranty period (typically 120 days) without additional charges.
  • Damage/Loss: Specialty lenses damaged within 90 days of the initial order will be replaced at 50% of the usual fee. There is no warranty for lost lenses.
  • Restocking Fees: Specialty, corneal gas permeable, hybrid, and scleral lenses returned in good condition within 60 days of the initial order may be refunded, minus a 40% restocking fee.

5. FTC Prescription Release (The Contact Lens Rule)

​​​​​Per federal law, your contact lens prescription will be released to you automatically once the fitting process is finalized.
